Op maandag is een proef met het vaccineren van pluimvee tegen de vogelgriep van start gegaan. Als onderdeel van de proef krijgen 1.800 kuikens een prik. Dit moet uitwijzen of de twee vaccins die in het laboratorium effectief bleken, ook werken in de praktijk.

De proef begint met zogeheten eendagskuikens, meldt het ministerie van Landbouw, Natuur en Voedselkwaliteit (LNV). Dat betekent dat de kuikens een prik krijgen op de dag dat ze uit het ei komen. Dat is niet ongebruikelijk. Er wordt al op grote schaal op de eerste dag gevaccineerd tegen de ziekte van Marek, een veelvoorkomende aandoening bij pluimvee.

De veldproef, die in opdracht van het ministerie van LNV wordt uitgevoerd, is de eerste in zijn soort in Nederland. Het moet de opmaat zijn naar een grootschalige vaccinatiecampagne voor pluimvee. Als de vaccins in de praktijk effectief zijn, dan moet er in het tweede kwartaal begonnen worden met een vaccinatiepilot bij een groter aantal pluimveebedrijven. De prik moet de dieren niet alleen beschermen tegen ziekteverschijnselen, maar vooral zorgen dat het vogelgriepvirus niet verspreidt.

Wageningen University & Research (WUR), Royal GD (Gezondheidsdienst voor Dieren) en Faculteit Diergeneeskunde van Universiteit Utrecht voeren de proef bij twee bedrijven uit. Het onderzoek loopt tot het derde kwartaal van 2025, maar de eerste resultaten worden ergens tussen april en juni van volgend jaar verwacht.

Omdat tijdens de veldproef vaccins worden gebruikt die nog niet in de EU zijn toegelaten, komt het vlees of de eieren van de kippen niet in de winkels te liggen.

Normaal gesproken verdwijnt de vogelgriep in de zomer, maar tussen oktober 2021 en juli 2023 bleef de vogelgriep Nederland in zijn greep houden. Onlangs is de ophokplicht die tijdenlang gold, ingetrokken. De afgelopen maanden werden miljoenen kippen, eenden en kalkoenen gedood om verdere verspreiding van het virus te beperken.

Dat meldt minister van Volksgezondheid Ernst Kuipers (D66) in een brief aan de Tweede Kamer. Het coronavirus veroorzaakt al lang geen crisissituatie meer, maar mensen in risicogroepen en ouderen kunnen nog altijd flink ziek worden door een besmetting. Zij krijgen de gelegenheid een herhaalprik te halen tegen het virus. De vaccinaties zijn aangepast op de omikronvariant XBB 1.5

De eerste herhaalprikken worden vanaf 2 oktober gezet. In Amsterdam zal dat op meerdere locaties gebeuren. De GGD maakt later bekend waar Amsterdammers terechtkunnen.

Uitnodiging per brief

Iedereen die dat wil, moet voor het einde van het jaar opnieuw gevaccineerd zijn. Wie in de zes maanden voor de geplande vaccinatie besmet is geraakt hoeft geen herhaalprik, stelt de minister.

De zestigplussers krijgen vanaf volgende week een uitnodiging per brief, de ‘griepprikgroep’ tussen de 18 en 59 jaar en zorgmedewerkers mogen vanaf half oktober zelf een afspraak maken bij de GGD.

Mensen in hoogrisicogroepen – met een ernstige afweerstoornis of het syndroom van Down – worden doorverwezen door hun arts. Zwangeren kunnen terecht bij de GGD en worden geïnformeerd via hun gynaecologen en verloskundigen. Bewoners in verpleeghuizen worden op de locatie gevaccineerd, mensen die niet mobiel zijn krijgen een prik aan huis.

De GGD’en hebben een basiscapaciteit van 10.000 prikken per week aangehouden, maar gaan dat voor komend najaar uitbreiden tot 300.000 vaccinaties per week. In totaal vallen ruim 7,5 miljoen Nederlanders onder de doelgroep voor een herhaalprik, maar vanwege de opkomst bij eerdere rondes rekent Kuipers op 4 miljoen prikken deze winter.

Digitaal dagboek bijhouden: eerste thuisonderzoek ter wereld naar migraine - RTL Boulevard

Hoewel migraine veel voorkomt in onze bevolking, weten we nog te weinig over de oorzaak, vertelt Mark van de Ruit, universitair docent biomedische technologie aan de TU Delft. "Migraine is een onvoorspelbare aandoening en dat maakt het lastig te onderzoeken", vertelt Van de Ruit. "Wij hebben in het verleden op verschillende manieren geprobeerd mensen met migraine in het ziekenhuis te onderzoeken, maar in praktisch alle gevallen dat patiënten naar het ziekenhuis komen, blijft de migraine uit. En een migraineaanval kun je ook niet even opwekken."

'Badmuts' met sensoren

Daarom gaan de wetenschappers nu op zoek naar antwoorden middels thuisonderzoek, als eerste ter wereld. De patiënten vullen thuis een dagboek in dat is ontwikkeld door het Leiden Universitair Medisch Centrum (LUMC). Daarnaast worden ze gevraagd om dagelijks kortstondig een soort badmuts te dragen met daarin sensoren die hun hersensactiviteit meet. Met informatie over de hersenactiviteit willen de onderzoekers de gevoeligheid van het brein voor migraine bepalen.

Volgens Van de Ruit zijn dagboekstudies ontzettend relevant in de wetenschap en worden ze al langere tijd uitgevoerd bij patiënten met migraine. "Door de dagboekdata te combineren met metingen van de hersenen, krijgen we een goed beeld. Zo kunnen we bepalen of dat wat mensen in hun dagboek opschrijven ook daadwerkelijk de trigger is", zegt de wetenschapper. "Stel: iemand schrijft in het dagboek dat migraine optreedt rondom de menstruatie, dan kunnen we door die informatie bepalen of het echt de uitlokker is."

Enorme toevlucht

Nederland is samen met België toonaangevend als het gaat om dagboekstudies, maar de methode is niet nieuw, vertelt Loes Keijsers, wetenschapper aan de Erasmus Universiteit. "Dagboekstudies bestaan sinds de jaren '60, toen ging het met pen en papier en kreeg je een pieper mee. Telkens als de pieper afging, moest je het dagboek invullen en na afloop met de post opsturen."

Wat wel nieuw is, is de schaal waarop dagboekstudies kunnen worden toegepast door technologische ontwikkelingen, zoals apps. "Daardoor hebben de dagboekstudies een enorme vlucht gekregen en levert het de wetenschap enorm veel informatie op", vervolgt Keijsers. "We zien dat ziekenhuizen de studies uitvoeren, maar de meeste dagboekstudies worden gedaan naar de mentale gezondheid."   

Ze is medeoprichter van een netwerk met honderd wetenschappers van de Erasmus Universiteit, dat onderzoek doet naar het dagelijkse leven aan de hand van dagboekstudies. Zelf gebruikt ze het ook in onderzoeken naar jongeren en social media en in onderzoeken naar opvoeding. "Dagboekstudies leveren de wetenschap verrassende inzichten op", zegt Keijsers. "Door een vragenstudie trek je vaak een algemene conclusie: zo werkt de mens. Maar door dagboekstudies leert de wetenschap dat elk mens een eigen recept heeft."

Ingewikkeld samenspel

Ook het onderzoek van Van der Ruit, zal geen eenduidige uitkomst opleveren. "Zeer waarschijnlijk zal het gaan om een combinatie van factoren en is er sprake van een ingewikkeld samenspel van allerlei zaken die maken dat de migraine optreedt." De wetenschappers hopen op basis van de verzamelde informatie uiteindelijk een app te kunnen ontwikkelen die voorspelt of je een verhoogde kans op migraine hebt.

Aziatische tijgermug duikt op in Noord-Scharwoude, bewoners ingelicht - NH Nieuws

De Aziatische tijgermug is gesignaleerd in Noord-Scharwoude. Om te voorkomen dat de exoot zich daar vestigt, gaat de Nederlandse Voedsel- en Warenautoriteit (NVWA) de mug bestrijden. In de directe omgeving zijn bewoners geïnformeerd.

Eind augustus is de Aziatische tijgermug op een adres in Noord-Scharwoude gesignaleerd, meldt de gemeente Dijk en Waard. De laatste jaren wordt het dier steeds vaker in woonwijken gespot. Dit seizoen is de mug al op 12 plekken in Nederland aangetroffen, meldde NOS vorige maand.

Bij de NVWA zijn er zorgen over de toename. De steekmug kan namelijk drager kan zijn van infectieziekten, zoals de knokkelkoorts of het zikavirus. “Daarom bestrijden we de mug. Zo kunnen we voorkomen dat de mug zich in Nederland vestigt”, aldus de NVWA.

Witte strepen

Tijgermuggen zijn kleiner dan de gewone mug en te herkennen aan hun zwarte lijfje met witte strepen. Ze steken vooral overdag. 

De gemeente stelt dat inwoners zich geen zorgen hoeven te maken over eventuele gezondheidsrisico’s. "In Nederland heeft de mug nog geen infectieziekten overgebracht en die kans wordt ook zeer klein ingeschat."

‘Maak melding’

Wie een tijgermug spot, wordt door de NVWA gevraagd dat te melden. "Probeer het dier te vangen of dood te slaan. Maak een foto en bewaar het voor eventueel onderzoek.”

Schuurmans deed onderzoek onder bijna 450 huisartsen en artsen ouderengeneeskunde. Daaruit blijkt dat de bereidheid onder artsen om euthanasie toe te passen groot is, behalve bij mensen met dementie. Artsen twijfelen over de waarde van een eerder opgestelde wilsverklaring. Ook hebben ze moeite met de inschatting van ondraaglijk lijden, een vereiste voor euthanasie. Zelfs als iemand vooraf heeft gezegd: "Als ik mijn geliefden niet meer herken is dat ondraaglijk lijden."

Muggenoverlast flink toegenomen, piek verwacht door huidige hitte - NOS

De overlast door muggen is na een relatieve meevaller aan het begin van de zomer flink toegenomen vanaf half augustus. Dat schrijven onderzoekers van de Wageningen University & Research (WUR) op natuurnieuwswebsite Nature Today. Ze verwachten dat de overlast nog verder zal toenemen door de huidige hoge temperaturen.

Dat het in het begin van de zomer relatief meeviel met de muggenoverlast is het gevolg van de lange periode zonder regen eind mei en begin juni, aldus de wetenschappers. Daardoor waren er zeer weinig broedplaatsen waar de larven van de steekmuggen zich in konden ontwikkelen.

Doordat het in juli en augustus flink heeft geregend, is daar vermoedelijk in die periode verandering in gekomen. De larven die eind juli uit hun ei kwamen, waren vier weken later volwassen, waarna ze overlast veroorzaakten.

Overlastpiek verwacht

De onderzoekers van de WUR baseren zich op meldingen bij het eigen onderzoeksproject muggenradar.nl, dat dit nu drie jaar monitort. Mensen kunnen hier aangeven hoeveel overlast ze ervaren en hoeveel muggen ze hebben gezien.

Door de huidige hitte verwachten de onderzoekers de komende tijd een piek in de muggenoverlast. Met hogere temperaturen gaan meer vrouwtjesmuggen actief op zoek naar bloed en de ontwikkeling van larven gaat ook veel sneller dan wanneer het kouder is, zoals normaal is in september. Door de regen van de afgelopen weken zijn er ook nog veel broedplaatsen waar de muggen zich deze nazomer kunnen ontwikkelen.

De verwachting is dat het aantal besmettingen met blauwtong oploopt. De meeste uit de nabijheid van de eerste besmettingen in Noord-Holland, maar ook uit andere delen van Nederland worden verdenkingen gemeld. In de meeste gevallen gaat het om schapen, maar er zijn ook zieke runderen gemeld.

Het blauwtongvirus dat rondwaart, betreft waarschijnlijk serotype 3. Dat is niet eerder zo noordelijk gevonden. Er zijn monsters gestuurd naar het Europees referentielaboratorium in Spanje. Demissionair landbouwminister Piet Adema verwacht dat zij in de loop van deze week zekerheid geven over het serotype.

Adema heeft de Deskundigengroep Dierziekten vorige week direct om advies gevraagd. Die kan geen betrouwbare inschatting maken van het totale aantal besmette bedrijven, de omvang van het besmette gebied of waar het centrum zich bevindt. Het virus waart waarschijnlijk al minstens anderhalve maand tot twee maanden rond in Nederland.

Op advies van de deskundigen doet GD retrospectief onderzoek op basis van gegevens uit melk- en bloedmonsters van de afgelopen maanden om zo het verloop in de tijd en de mate van besmetting in beeld te krijgen. De uitkomsten van dit onderzoek worden in de loop van de week verwacht.

Geen aanvullende vervoersbeperkingen

Er komen vooralsnog geen aanvullende vervoersbeperkingen. Adema stelt dat het effect daarvan beperkt is, wanneer er sprake is van een groot verspreidingsgebied. De omvang van het gebied is nog niet duidelijk. Dat wordt nu in kaart gebracht.

Op het moment dat daar meer duidelijkheid over is, beoordeelt de minister of en welke aanvullende maatregelen effectief kunnen zijn. Het blauwtongvirus wordt overgebracht door knutten. De verspreiding van knutten beperken noemt Adema niet mogelijk.

Veehouders krijgen het advies terughoudend te zijn met dierbewegingen. Het risico bestaat dat niet alleen besmette dieren, maar ook besmette knutten met een transport worden verplaatst. Verder leert de ervaring van de uitbraken in 2006-2009 dat opstallen van runderen in goed geventileerde stallen enigszins beschermend werkt.

Zuid-Europa

Blauwtong virus 3 is tot nu toe in de Europese Unie alleen vastgesteld op de Italiaanse eilanden Sicilië en Sardinië. Hoe het virus de sprong naar Nederland heeft gemaakt, is nog onduidelijk. Tegen het virus is geen geschikt vaccin beschikbaar.

Adema noemt dat een tegenvaller voor veehouders. Vaccinatie is namelijk het belangrijkste instrument om blauwtong te voorkomen. Er vinden nu gesprekken plaats met farmaceuten om de mogelijkheden van vaccinatie te verkennen. Blauwtong is niet overdraagbaar door direct contact tussen dieren en ook niet besmettelijk voor mensen.
